  3. I understand the questions in the PEP medical form and have answered honestly. Any inaccurate or false information could potentially be dangerous to my health and safety.
  4. The medication that is prescribed is for my personal use only. I understand the side effects (including potential liver and kidney damage) and effectiveness of the treatments.
  5. The medication will only be effective if taken as prescribed.

  8. I understand that I still need to urgently visit my local clinic to do blood tests, including an HIV test, and possibly receive a hepatitis injection if needed. 
  9. I understand that my local clinic needs to monitor me for potential complications such as side effects, sexually transmitted infections, hepatitis, pregnancy, and organ damage (liver and kidneys). Any wounds must be thoroughly cleaned with soap and water and assessed at my local clinic as soon as possible.
