Both are true, roughly. But which one actually suits you depends on how often you want sex, how your body handles the drug, and what a doctor finds when they check your heart and your other medicines. How tadalafil works

An erection is really a plumbing event. When you are aroused, nerves release a signal that relaxes the muscle lining the blood vessels in the penis. Blood flows in, the tissue swells, and the outflow gets squeezed shut. That is the erection.

An enzyme called PDE5 is what ends it, by breaking down the messenger chemical that keeps those vessels open. Tadalafil blocks PDE5. With the brake off, it becomes easier to get and keep an erection when you are aroused.

Two honest caveats. The 36 hours is a window of easier erections when you are aroused, not a 36-hour erection, and it is not a promise every man hits the full window. Never mix tadalafil with nitrates. Nitrate medicines are used for angina and some heart conditions. Common ones include glyceryl trinitrate (GTN spray or tablets under the tongue), isosorbide mononitrate and isosorbide dinitrate. Both tadalafil and nitrates lower blood pressure, and together they can drop it far enough to be life-threatening. Because tadalafil lasts so long, the caution window stretches too.   • had a heart attack or stroke recently, or you have a heart condition where you have been told sex is a risk
  • low blood pressure, or high blood pressure that is not controlled
  • serious liver or kidney problems

  • a blood cancer such as leukaemia or multiple myeloma, or sickle cell disease, which can raise the risk of a prolonged erection
  • a physically deformed or curved penis that makes erections painful

Side effects

Headache is the one most men notice, somewhere around one in ten. After that: facial flushing, a stuffy or blocked nose, indigestion or heartburn, and back pain or muscle aches. The back and muscle aches are a bit of a tadalafil signature. They tend to show up 12 to 24 hours after the dose and usually settle within a day or two. Simple pain relief often helps, but check with a pharmacist or doctor about what is safe with your other medicines.

Then there are the rare but serious ones that mean stop and get help.
